The Time Has Come quest in Kingdom Come Deliverance 2 Mysteria Ecclesiae: Walkthrough
After completing To Dust You Shan't Return and Seek and You Shall Find quests, the monastery will be out of quarantine. This means you can now easily move to any location.
Speak with Ludolf and acquire your armor

First, navigate to the knights of the order camp in the far west and talk to Ludolf.
Then, take the armor and helmet from the camp for yourself. These can be helpful during the final stages of the quest.

Where to find King Wenceslas' medical records

As your clue for finding King Wenceslas' medical record is "the records are behind the scales," you have to go to a specific location to acquire it. Navigate towards the Abbot's chapel inside the monastery, which you can enter using the red door.
Now, go behind the Altar to pick up King Wenceslas' medical records.
How to forge King Wenceslas' medical record

The main task is to learn how to forge documents, and then apply that method to make a fake Wenceslas' medical record, which you can show to Zacharias.

If you have the original Wenceslas' medical record, go down to the cellar and get to the scriptorium, where you can forge a fake one. However, you need to read the real records first from the Quest tab in your inventory.
Then, choose the option to make fake records and select the following prompts:
- Without addressing
- Head and stomach pain
- Less drink
- Infertility
- Healthy lifestyle
The final confrontation with Zacharias (the endings of Mysteria Ecclesiae)

Firstly, it's recommended that you equip good weapons and armor, as the next part will involve some fights. Now, go to the church area and wait for Zacharias.
Subsequently, you can opt for one of the two DLC endings depending on the course of your actions. The only way you will get the bad ending is if the forged medical record is not done appropriately.
Here are the details:
The good ending of Mysteria Ecclesiae
After waiting for Zacharias, choose the following prompt while talking to him:
- Give the forged records. (if forged correctly) / Give the real records.
When the next cutscene ends, you must defeat one of Zacharias' men.

Then, go down the crypt, continue southward, and at the end of the pathway, go through the door in the southeast direction.

The subsequent door will be locked at this time. However, an enemy will immediately come at you from behind. You must defeat and take the key from him to unlock the door.
You can also take the painkiller brew from him, which you can give to Zacharias later for an additional dialogue.
After going through the door in the south, continue forward, and go west to find another enemy. Defeat him, proceed forward, and exit the cellar using the stairs.

Afterwards, a cutscene will begin, and when it ends, you have to defeat Zacharias. Then, choose any dialogue to learn about his motive and get the antidote for Albich.

Through this good ending, Albich will fully recover from the disease. Then, he will be able to save the rest of the sick people at the monastery and bring peace.

The bad ending of Mysteria Ecclesiae

After waiting for Zacharias at the church, he will ask you for the medical record. If you give him a forged one that is not made correctly, he will laugh at you. After that, proceed in the same way as mentioned in the good-ending part and defeat Zacharias.
However, this time, he will break the antidote vial, which was supposed to save Albich. As a result, Albich will go blind and won't be able to save the sick people, and many will die.

Speak with Albich and Abbot, and leave the monastery

When the final cutscene ends, talk to both Albich and Abbot. You can choose any of the options while talking to the former. While talking to Abbot, if you ask him what will happen to the monastery and say the relic will help, don't choose "It makes no difference now." It will result in Reputation loss.
After the conversations, exit the Sedletz monastery using the western gate. With this, Kingdom Come Deliverance 2's The Time Has Come quest and Mysteria Ecclesiae DLC are completed.
You can return to the Sedletz monastery whenever you want. However, be sure not to carry any stolen items, and give the guards 300 Groschens as a donation to enter.
